Each year, athletes in our district are required to submit proof of a physical signed by a physician in order to partcipate in sports. Physicals are required every 2 years and in non-physical years, an alternate year card must be signed by a parent. After athletes complete the physical and appropriate paperwork is completed, including physician signature, date of physical and birthdate of athlete, the paperwork (or alternate year card) must be turned in to the Athletic Director at the middle schools or the Athletic Trainer at BMHS BEFORE students will be allowed to participate in a sport.
Other places in Beloit to obtain a physical if you are unable to schedule with your child's regular doctor are: Beloit Area Community Health Center (361-0311), West Side Medical Center (363-7510), Occupational Health and Sports Medicine Center (364-4666) and, as a last resort, Convenient Care (364-2410). These are usually covered by insurance or are provided for a fee.
Please make sure all physical/alternate year paperwork includes the following information: 1) Student's First and Last name, 2) Birthdate, 3) Grade, 4) Date of Physical, 5) Signature of Physician (on physical card only) AND 6) Signature of Parent/Guardian (on BOTH physical and alternate year form). If any of the above mentioned items is not on the physical or alternate year card, it will not be valid and will be returned to parent.
